Dear Maggi,
If you read carefully or read between the lines of the mail that your "boss" sent you then you will find that on the one side your boss has shown empathy for your accident and at the same time expressed his displeasure for your non-communication. Therefore, this mail can be construed as "acceptance of resignation" mail and at this stage no further correspondence is required. Once you get all your dues, send him thanks giving mail for having settled your dues in time.
Before quitting the company, obtain the clearance from the departments and make sure that no dues are outstanding against you. Ensure that company property, if allotted to you, may be returned. Keep the second copy of the handing/taking over certificate with you.
Thanks,
Dinesh Divekar

Thank you Dinesh Divekar for responding...thank you so much...
But my boss will expect a reply from me...

I would like to send reply to my boss like this.
Please check is this okay...or do i need to add anything more....

Respected Sir,

Thank you for accepting my resignation. Sorry for the late update about my accident.
I had already informed about my accident and health condition to Mrs. Krup(Supervisor). Its my mistake that i should inform you before about my accident i applologize for the late update.I thought to rejoin once i recovered that is why i did not inform you. But my illness will require extended treatment and recovery, I have been advised that I must rest full time to me to recover fully from my illness. So i have decided to resign my job.

I would finally like to thank you for the experience you have given me working with DataSoft Networks and I would like to wish you all the very best of success for the future. I feel that DataSoft Networks has a very bright future and I am sorry that I am not going to be part of that.

Best Wishes.

Yours sincerely
A.Radhika

I would like to include a line ...saying thanks for settling my due salary....

Dear Radhika,

Your draft is little wordy. It needs to be made concise. Following is the revised draft:

+++++

Dear Sir,

Received your mail confirming the acceptance of my resignation. Thanks for the acceptance.

I had sent mail about accident and my deteriorated health to Mrs Kurup (Supervisor) on _______ (date). Regret to note that this mail did not reach you. Incidentally I had not marked CC of this mail to you.

I had not anticipated my delayed recovery. Therefore, in order to curtail this prolonged absence, I have taken decision to resign from the job.

I had not anticipated that my separation from DataSoft Networks will happen under these circumstances. Nevertheless, my stay was smooth and I express my deep gratitude to the staff of DataSoft Networks in general and our team in particular for providing me assistance and cooperation. I am thankful to you for providing me all the necessary support and guidance wherever necessary. Though my tenure at DataSoft Networks has been ended abruptly, I will cherish its positive memories for a long time.

Warm regards,

A. Radhika

Hope this satisfies your requirement.

I think your letter is perfect for the purpose. And since it is in the manner and pattern you normally use, it makes more sense than to take a format from the net or made by someone else, however much it is a better format. If you use a formal tone, it looks like you are replying as a formality. If you use your own draft, it looks like you are genuinely writing what you feel. So stick to it. your language and drafting skills are pretty good, so no worry on that account.
If you wish to add a line thanking for the clearing of salary without notice, do add that line, again in the pattern you always write.
You may also add a line saying perhaps once you are well (which i assume will be a long time), if there are positions available, you will be considered for the same. That would be in line with the thought put in the rest of your letter. Ofcourse do it only if you would really like to rejoin
