In form 10C & 19 you are only entering the personnel details and employment details. Apart from that you are declaring Date of Joining and wage details. Wage details shall be given from HR or they will enclose copy of Form 3a Instead.

Please go through the caption form.
Fill up the employee details, PF account No. etc.
Fill up the wage details, you will get it in Form 3A.
Take the signature of the employee and also on 1Re Revenue Stamp.
Get it signed by Your authorised signatory of the company.
Submit it after 2 months from the date of resignation of the said employee.

Please find enclosed claim form for your reference, you can fill and signed accordingly and get attested/signed from previous employer and sent directly to the concern EPF office.

Hi,
I have worked in one organization for 3.8 years, during which I received 25427 as Pension Fund. I have since joined another organization and transferred the EPF amount from my previous organization to my new EPF account.
Although I have successfully transferred the amount to my new EPF account in the new organization, I have not received the EPS amount of 25427.
Could anyone suggest how I can get this amount transferred or withdraw the EPS amount?
