What is the purpose of the survey? How many responses are you expecting? Are you surveying your own staff or are you expecting members to respond? Do you want to conduct any statistical tests or are you going to just use descriptive statistics?
There are more questions that need to be answered before people can give you suggestions. Also, please proof-read the questionnaire to eliminate errors which a "Spell & grammer check" will not identify.

I am a bit puzzled. Why are you doing the research? If you have already collected data, may we please know what data you have collected? Have you done any literature search to see what others have done, how and why that way? What are the project's objectives and your hypotheses? We need to know all those before we can guide you how to design a questionnaire.
