Actually, I am an HR Trainee. I want to know how to apply for ESIC for employees, what documents are needed for ESIC, and finally, how to claim or use those features. Could someone please explain my doubts?

During the payment, you need to generate the ESI number of the employee. During that time, you need to enter the details of the employee. Those details you can collect from the employee through Form 1.
Take a printout of the Temporary Card and give it to the employee to check the details and make the necessary corrections and provide a fresh printout. Ask them to attach a family photo (small size) and return the form. Attest the photograph with the Authorized signatory (with seal) and return it to the employee.
Advise the employee to go to the nearby ESI Branch Office along with the temporary card for Biometric Registration. Once it is done, within 30 days, the employee will receive 2 ESI permanent cards. Advise the employee to give one card to the family and ask their family members to undergo the same biometric registration process by holding the ESI permanent card. (The employee does not need to accompany them).
Claim: The simple procedure is that the employee should visit the respective ESI dispensary and get approval for medical reimbursement or leave salary (or both).
For medical reimbursement, the employee should obtain the referral letter before receiving treatment or within 2 days of the admission date. If the employee receives the referral letter before treatment, then all payments will be made directly to the hospital by ESI. If the employee gets the referral letter after admission, he/she can only claim for bills after payment. During the claim, the employer should provide a letter stating that they have not provided any financial support to the employee during treatment. The employee should submit the referral letter, letter from the employer, and medical bills to the ESI Branch Office.
For Leave Salary, the employee should obtain a leave letter from ESI. They will provide the leave letter only confirming the sickness. After the leave, the employee should submit the leave letter to ESI. ESI will request the employee to obtain a signature from the employer on a leave confirmation form. The employee should submit the filled form to ESI along with the employer's signature. ESI will pay the leave salary to the employee's bank account.

If a worker or staff member's gross salary is above 21,000, and they worked for only 10 days in April earning 8,000, are they eligible for ESIC coverage? For example, if the gross salary is 17,000 + 4,900 (HRA) + 1,095 (other allowances) = 23,095, and the worker only worked for 10 days receiving 8,000, should they be covered under ESIC or are they exempt from coverage?
