I am sorry, but I am not able to understand your query. ESIC refers the IPs to private hospitals if ESIC hospital couldn't provide the required treatment. Were you referred to a private hospital by ESIC, or did you go on your own and are now seeking reimbursement from ESIC? The forms you are talking about are IRDA required to be filled by any insured to claim the expenses. Form A is to be filled by the insured, and Form B by the treating doctor. I couldn't find the link even in my answers, but I hope this solves your query.
