Hello Ashwini,
ESIC Act applies to all employees whose Gross salary is less than or equal to Rs. 10,000/-. Form 01 (Declaration Form)needs to be filled by the covered staff at the time of joining and should be submitted along with 3 copy of Return Of Declaration Forms 03 separate for Males & Females.
Half yearly return needs to be submitted with the Branch office along with the copies of submitted challans.
