Dear Sir/Madam,
At when I, Rahul N K visited to ESI hospital and esic office today for a super speciality treatment reference letter, They (Esic officer in Chitradurga,KA) have rejected due to the reason of '' No 2 year continuous contribution'' but I have left the job on August 2015 and joined another company on September 2015.
But in the new company they have generated another ESI NO 858618697 and made the payment for the month of September 2015 and October 2015.
After this I requested to the ESI number to my previous ESI no 5343118454 and contribution continued in this number from Nov-2015.
Hence there is a 2 month gap in this ESI number.. Due to this reason I am unable to get esi benefits..can anyone guide me to resolve this issue.

At the time of joining you must provide them your old ESIC no. to new company and it is also responsibility all company to get all new member their old ESIC and UAN if they have but do not mind you tell your old ESIC no. to your company HR. Because they will insert your old ESIC no. in current month and say them that please make a letter to ESIC for transfer new ESIC amount on the old ESIC number.
