Please let me know whether May 1st should be declared as a compulsory Holiday for all companies. or whether it can be a discretion of the management to exclude it from list of holidays?

Greetings.
As per National and festival holidays act, the following national holidays has to be declared as Holiday in any establishment.
1. Republic day ( 26 th Jan )
2. May day ( 1 st May )
3. Independence Day ( 15 th August )
4. Gandhi Jayanthi ( 2 nd October )
Any employer who contravenes the above, will be penalised as per Act.

I am from an International Call Centre.
Here we do not give indian holidays, we give employees US holidays...
So in this case is it still mandatory to give the employees, US holidays + indian holidays?

That depends from company to company, some companies give indian holidays and some give holidays according to usa, but that also depends on the employment contract signed between an enmployee and employer, it mentions whether usa or indian holidays will be given.

act 01 may be icluded in list of holidays,which will be applicable in Tamil Nadu.As per national festival and holidays act 01 may is not national holiday and it is not compulsory to add this is list of holidays.
however national holidays are fix ie 26 jan,15 aug and 02 oct but festival holidays including 01 may is to be decided by the management with the concurrence of workers.

If employees working for USA/UK shifts and they follow USA/UK holidays.. Example if they work on 1st May 2017.
Is there any approval forms by Karanataka Labour Office. Where we need to submit letter and get sign from Labour Office. as our employees works for USA/UK and they will follow USA/UK Holidays.
Please let me know if any form where i can get sign from Karnataka Labour office.
