Its not mandatory but still we required to have smooth operations towards HR activities in addition after hiring the employees towards employee replacement, people will question you that who had given approval on the same like salary, replacement.
To avoid above issues better to have employee requisition form and get the sign from MD or CEO or COO

It all depends up on the size of the company , it' s work culture, systems and methods.However as a sound procedure which works on the principle of accountability, it is always better to have a record of manpower planning like which department requisitioned the manpower and why for future reference ilike HR audit etc.

Hi............ ask the dept. to send you the mail what kind of candidate they require and on what qualifications and what age..... and experience...... Better to take for your records and accordingly you may go for searching the candidate.

It depends on the policies and procedures make by organisation for manpower requirement.
It may not be necessary that if a employee has left the organisation there would be a replacement. You need to show the management that the replacement is necessary and hence most of the organisation have employee requisition form which have details to justify the need of resource of the department as this is a cost to the department.
This also help to plan the budget and cost by respective department and HR too to search the right candidate for the department as required.
