The Q12 questionnaire can be a basis of any employee engagement survey. One can add more questions and tweak existing questions to suit your requirement.

You can outsource this job to a Consultancy who do this kind of work. You can have paper and pencil survey or an online survey. In my opinion online survey is better as processing the results is much faster and it ensures a high level of confidentiality.
