Report for the Human Resource generally involve all data that are of relevance to the activity of the whole human resource of the organisation for the given month. You are working on HR and in this case, the organisation should design a standard format for your report. HR reports varies from different organisations as the HR of different organisations varies also in terms of principles, processes, systems, size, etc.
