I need some guidance regarding the format of a payslip. I work in a small company with less than 10 employees in the Mira-Bhayandar area of Mumbai as an IT Support Technician. My employer informed me that he does not deduct TDS, PF, or any other benefits from the employees' salaries. Instead, he pays a fixed monthly salary to the employees, which in my case is 20K, summing up to an annual salary of 2,40,000. I need a payslip format without TDS and PF deductions. The only deduction my employer is willing to make is the Professional Tax (PT) of 200 per month, as is mandatory in Maharashtra. I heard that Medical and Conveyance allowances are not required for the years 2019-2020-2021.

Could anyone help me with the format of a payslip for the specified years, considering my monthly salary of 20K and annual salary of 2,40,000, and no deductions other than PT (200)? What are the mandatory components to be included in a payslip as per the standards in Mumbai, Maharashtra? I need to submit this to the CIC for a Visa application, and they are very particular about the payslip. Should my payslip show a gross pay of 20K, deduct PT -200, and then show my Net Pay as 19800? Is there any specific format to be followed? Any help would be appreciated. Thank you.
