When raising a post on a public forum, especially when seeking advice from seniors, a member is expected to provide complete information. This includes details such as when the offer letter was issued, the designation offered, the date of joining, reasons for the selected candidate seeking an extension to join, instances of backtracking from the commitment, details of correspondence with the candidate, and more.
Without this information, it becomes challenging to draft a letter effectively. Additionally, when preparing the draft, you could have shared it here for review by seniors. While some HR managers may rush to provide drafts without adequate information, spoon-feeding in such cases does a disservice not only to the individual seeking the draft but also to the HR community at large.

In the future, in the offer letter itself, please always include the following point:
"This offer is valid till ____ only, and your failure to report to duty on or before ___ will lead to the cancellation of this offer without any further communication."
For the current case, you may write an email as follows:
"With further reference to our offer letter dated ____ issued to you, please note that our offer letter stands canceled with immediate effect, as you have failed to report to the office on ____ as committed by you. Acknowledge receipt."
Please acknowledge if you have any questions or need further clarification.

Did the candidate inform you of his inability to join your company? If he has informed you, there is no need to send a revocation letter for the offer. Always insert a line in the offer letter so that you would not have to send a revocation letter:
"This offer is valid till the date, which stands cancelled as you failed to join by the date without making any further communication to you."
