There is a lot of information to be gathered and filled in department forms, such as service span, present salary (basic and gross), reason for death, death certificate, ESIC/EPFO account present status, and their validation with Aadhaar. KYC update is required in both departments.
First of all, the family has to visit the panel doctor of ESIC or the local dispensary to seek forms for the claim of funeral expenses. These forms need to be signed by the employer and submitted for the claim process. If there are minor children of the employee enrolled in the card, ESIC will issue the next form for the claim of compensation. If there are no minor children, then ESIC is limited to covering medical and funeral costs only.
Under EPFO, Form 20, Form 10D, and Form 5 IF have to be submitted with the requisite documents (as mentioned on the form itself) by the registered nominee. If KYC and e-nomination are completed, the process will be smoother.
