Your question is a little confusing. After conducting Training Needs Identification (TNI) or Training Needs Analysis (TNA), the training programs are decided. Thereafter, before the trainer conducts the training, he/she circulates questions among the participants or to the HOD. This questionnaire is called a Pre-training Questionnaire (PTQ).
Nevertheless, your question insinuates that you wanted to do TNA. Therefore, please confirm what exactly you want.
