Would appreciate if you all can suggest some changes and How do I go for the assessment on the basis of following feedback form.

Hi Archana,
I have gone through your feedback form, its good and very informative.
But i would like to add this.
Name of the Program
Date of Commencement
Faculty Name and Topic... will give you a clear picture about the whole Training program. (If the training is on continuous process)
About the rating scale, do you feel it is necessary to mention Good, Not Good in each line. I feel it will confuse the participants. So please check from your end.

No doubt the feedback form is good. But what we usually do here is - for the every response given by the participents......u might be analysing the outcome right?.... i am moving a little bit ahead....
we need to have a set of TODO list which will let u know as to - what has to be done for a perticular outcome.
Eg:
Thum rule : If preparation rating is below or = 2. Then trainner need to be briefed on outcome.
Outcome : Average prepration rating is 2.
Ans : Approach the trainer for better prep of course materials/speech.

your feed back form is good but the overall evaluation of venue is not required this factor does'nt play a very important role, I would rather suggest u to remove it.
On the other hand performance after training prog. is important hence i am posting this format for evaluating employees perfomance after training, this can be done in two ways one immediately after training prog. by framing a questionnaire and making participants answer it so that maximum retention and attention is amde possible. other way is feedback from hod which is done after a week.
Below given format is based on number of questions hence percentage may vary
