Hi Anil,
Thanks for sharing the training feedback format. It is even more important to take a feedback from the Senior of the employ to assess the effectiveness of the training given. As a result of training the capabilities of the employee should be improved that is the very purpose of giving training. Obtaining a feedback from the employee about the training is a mere formality and does not serve the purpose of giving training.
So i very humbly suggest you to modify the form suitably so that the effectiveness of training is assessed by the senior person
