Interview rating form differs from company to company u must understand what competency or waht characterstics your looking in candidate for example
1. Apperance & Presentation
2.Communication
3. Maturity
4.Education
5 level of Professional Knowledge
6.Academics
7.Extracuricular activities
8.Reasons for Planning for a change
9. Industry Knowledge
10 Knowledge abt the company
11. Past Experience
I guess this characterstics would suffice for any Preliminary level of interview.
If you can find out all this from the candidate i guess you can make a reasonable judgement .

It is important to know the competencies of the position. There are a lot of Assessment Forms nowadays but the most useful and more appropriate is a competency based assessment. Quite busy but would glad to give you more inputs soon.

It is rightly said that assesment sheet differs from company to company and better to be competency focused.
Even our CHR posted a format of interview assesment sheet, you should check that to, it will guide you much better.
