Death certificate is to be issued only by the competent authority i.e the Registrar of birth and death for the area where the person died and normally it is the Local Body Administration. If you are the superior of the deceased, ask his family to submit the death certificate for the purpose of reporting onwards as well as to distribute the terminal benefits of the deceased employee.

Dear Mr. Vijay,
from your post it seems that you need to give service certificate to the member who has death while in service.
All the column in the regular service certificate will be same. Only reason of leaving will be death in service.

Are looking for a death certificate to be issued by the employer to the employee family , is that what you trying to convey.
