How do we assess "willingness to take initiatives"? Since it is a subjective question, how do we rate someone on this point for the behavior skill matrix?
I mean through a questionnaire format with situational questions or by his/her supervisor's perception?

I am working with a pharmaceutical manufacturing company. We need to prepare a skill matrix for all levels of employees, i.e., from operators to Site Head posts. Kindly guide me on how to prepare a skill matrix - what points to be taken into consideration for preparing the matrix.
