At any point in between the contract period, you reserve the right to terminate the contract agreement even without any reasons. Once you resign, the contract gets terminated, and your exit formalities have to be followed. If the company fails to react, appoint a lawyer and exit the company. It's damn easy with the help of a lawyer. Not so difficult.

Advocate is right; you can take legal action. However, beware; even the lawyer cannot prevent your employer giving you a true exit certificate on the lines I have mentioned.

I would advise that you come clean and say that you found a better opportunity. Given that you are ready to pay the penalty, the team should provide you with a normal relieving letter. If you continue to stick to your 'medical' reason, the company may issue you a normal relieving letter but could complicate your background check.

I strongly second Ayush Pal. Considering your medical certificate, assume that the company gives you a relieving letter with a clean chit stating relieved on medical grounds. However, if your new employer asks for your relieving letter or a BGV, you will be in a fix. Therefore, it is advisable to state the exact reason for leaving the job. If the company is good enough, they will relieve you as per the termination/penalty clause. Wish you all the best.
