Well, you need to consult the HR dept. asap to correct the information. Event the process outsourced to third party still your HR dept can only help you to correct the submitted information by asking the third party to do so and in this way you can correct the information to avoid any further issues regarding pf withdrawal in future.

Dear Shilpi,
In current practice, All employer has to fill up / updated missing details like Name, DOJ, DOB etc through E-Seva(online) so please contact to your employer(Outsource Agency) and give application for changes so he can directly update in missing data file that PF department will change accordingly.
Else, Your employer will submit the application to PF department separately for the same.
