· Letter Head
· Address Seal & For Seal
· Ownership details – Partnership deed – MOU/MOA - Proprietor’s Name, Father’s name, Address, Age,
· Address Proof, Rental Agreement or Sale Deed.
· Rent Agreement/ Lease Deed/ Address proof/Ration card
· License – Factories / shop / contractor license
· Photocopy of Current A/c No. statement
· Photocopy of PAN No. card
· Commencement of Production Certificate from DIC
· PGST/CST Registration copy
· Service Tax Registration with Central Excise Copy
· First Month to till date - Attendance & Salary registers’ photo copy
· First Invoice Copy
· Trial Balance (for the estt. Existing more than one year)
· 3 Labour Contract Agreement from ESI covered units & Last month bill copies attested by the company along with their ESI Code No.
· Bill copy raised to the companies duly attested by the clients companies.
· List of employees

· ESI & PF,Time Limit – 45 to 60 days
· Challan copies of remittance of contribution by the Principal employer (in case of contractor)
