hi this is very good and compiled information on appraisal format as in our company we have a very lengthy document which really takes a hell lot of time......

Hello Abhi,
Just a suggestion from my side: You can assign weights to the various parameters, this will help you in attaching relative importance to the various job responsibilities, I am sure you are aware of this weighted appraisal system, however, you can also do without the weights.
In the "Critical attributes" section, you have mentioned" Job Knowledge, Skills, Competencies & Application".........what competencies you are referring to? ....it would be better if you could have been a little more elaborate.
You could also have broken up the "Leadership Skills' into various sub heads ...for measuring various leadership aspects.
I am attaching the appraisal form that I designed for my co........you can take a look.....you can give me the feedback......
