After going through the forms create one according to the project industry you are working on and ask the employees to fill it up.

I am working in an NGO that focuses on women's rights. It has 164 staff members in different states. Additionally, for those employees who have contributed their time and effort to campaigns not in their Terms of Reference (TOR), how can I recognize their contributions?
I am the first HR person in this organization, and it is appraisal time. We do not have any HR software to manage HR processes, and I have never conducted an appraisal before. I need help with the performance appraisal process.
