Dear Arunaa,
What kind of verification you are seeking from an employee, Like his ex-employer verification, his temporary/permanent residence verification, his police record verification or his eligibilities/total experience verification. The format can be made according to your needs, there is no as such fixed formats are used for the same.

Hi Arunaa,
There is no format as such, an employee can be verified from the following ways on the required parameters:
Compensation : His last salary slip along with his bank statement copy.
Experience : His appointment letter and copy of resignation/relieving letter from his ex-employer. (or his all ex-employers)
The other things like his loyalty and his performance can be accessed by his ex-colleagues/ superiors of other related people of the industry, but should be done in a very confidential manner as it can effect our own reputation also. I've doubt if any employer can give this kind of verification to his ex-employees on a format.
We have to create our own network to gether this kind of information from the industry which always be helpful to take decision to the HR depot. and other related authorities.

Hi Aruna,
I am a Recruitment Specialist, in a MNC in Hyd, hmmm....I can help you in this regard as I too, do ex-employee verification.....
What you can do is ...prepare a format (WORD or EXCEL) mentioning or say asking for the following:
1. Previous Employer
2. Contact Person Name with Designation (can be any co-worker, peer, project manager, HR, any superior...)
3. Contact Details (mail id or number or office board number)
And add a statement "that this is highly confidential and will be used only when the candidate joins ......(your company name)".
This form can be given to the candidate to fill up once they accept the offer made by your company.
After the candidate provides this info to you , you can call up the person provided in the form and ask wat you want to know or mail them accordingly.

Hi Minakshi,
The 3 points u have said is good
But the Candidate who is going to join in my company shud not come to know that we have verified hin with his x - employer
So am not in need of asking the person to fill... Am going to verify him with his experience certificate or with the help of his resume

Hey Aruna,
You can also directly use the official numbers mentioned in the docs provided by the candidate like salary slip or offer letter and ask for HR's help in order to do an employee verification , but do it once the candidate joins your org.
But the format mannerism is recommended as this awares the candidate and lesser chances that he fakes and provides wrong info.

Many a times the company doesn't provide this info untill unless yu disclose the person's EMP ID and / or disclose the purpose of asking for such info....

Dear Aruna,

Hope things are fine with you.

Its a very good idea of have reference check for the employees joining in the organization.

Are you planning to do ilt for all the employees joining in the organization or its only for managerial or project lead level?

It is little difficult to do reference check from his previous employer before one joins the organization as the person handling such task has to be very tactful . The employer at any point should not be told that the person whom you are verifying is exploring opportunities outside as he might be in trouble later. I had similar experience in my company wherein i received a call from Employee verifiction firm , they were verifying the details of an employee who was still working with us.

For Employee verification you can also take help of various Backgroung screeing firms you can help you verify the details of the previous employer.But you need to be specific when you are verifying the details of an employee before he is relieved from the previous employer.

You can use the below mentioned format for an employee who has been already relieved from previous employer:

.1) Name: XYZ



2)Employee code: XYZ



3)Period of employment: XYZ

4) Reaso for leaving:

5) Can be consider for Rehire:

6) Attitude and integriy issues:

These are just some of the points. You can add on as per your requirement.

Hi Arunaa,
Sending you a simplest format that you can have and is open to customisation. There are two parts involved in the format. The first part is the reference check that you get done fron the candidate's past / ex immediate bosses or colleague sand the second part from HR dept. I am following this format for my company and very much comfortable to use it ( may be because I have designed it)while doing a telecon with refree or sending a mail to them.
For me the first part is of more of importance because you are getting information over the candidate on both personal & professional level.
We don't call it as employment verification since its a level; higher than that since you are only not verifying the information given by the candidate but also going ahead checking his attitude, honesty, commitment.............................all that which are really of concern, hence its a entire process of running a refernce check.
