Dear SSM,

Some time back, I had posted about the roles of supervisors in accident investigation. I am reposting the same PowerPoint presentation again; maybe it will be useful to you. Similarly, I remember a thread on why-why analysis that should also be useful to you.

Thanks & Regards,
Sudhir

  • Dear Sudhir,

TRC stands for Total Reportable Cases. PAER - P for people; A for asset (or property); E for environment; R for the reputation of the organization. These are the four main areas that are affected by the occurrence of an accident or incident.

  • Dear Avinash,

See the attached reports. Every company has a separate standard Accident/Incident report format. You may find numerous documents on the web; I obtained these from the web only. Please review the attached Final report of the fire accident to observe the various investigation methods and styles.

    Keep in touch...

(Fact Check Failed/Partial)-The user's reply is incorrect. Incident investigation minutes should focus on the facts of the investigation, not on employees' performance unless directly related to the incident. It's crucial to adhere to confidentiality and legal requirements.
