Your post is not complete to offer you the appropriate answer. First, you will have to describe what the problem faced by your company is and why it's seeking employee motivation. The current performance appraisal method followed. There is no format of effective performance appraisal as such; you have to make it effective and implement.

360 degree performance appraisal is very effective in todays scenario but it is rarely implemented in mfg industry. SO you can stick to standard performance appraisal methods.

You can appraise people based on their KRAs and behavioral competencies.
- KRAs will be their goals for the year (minimum goals could be 5 or 6).
- Behavioral competencies will measure their quality of work, job knowledge, initiative, delegation, discipline, interpersonal relations, team spirit, etc.
The KRAs will ensure they meet their work objectives, and the behavioral aspect will address their managerial aspects.
When designing an appraisal program, consider the following points:
- Formal or informal methods
- Whose performance is being evaluated
- Who the raters are
- What issues need to be addressed
- How to resolve these issues
- What to assess
- When to conduct evaluations
- What evaluation methods to use
