I would suggest a simple way: Google it, and you will find a lot of exercises to develop these skills. Note down all these exercises and think about which ones would suit you the best, then start doing them. Along the way, try to develop a passion for it. When you develop the passion, everything will become easier. The only thing you need is patience to gain these skills, as they will take time and won't come all at once.
I was very bad in English a year ago. After following the aforementioned method, I could see a considerable improvement in my language skills.

Well said. I completely agree with you because I do have the same problem. It's not a question of good English or proper grammar. The main issue lies in framing the correct words to form a sentence during speaking or written communication. This is where we need help.
