Hi Guys, What is that I am looking for is a Pre- Training program Questionnaire to be filled out by the employee for the setting of expectations from the training program and then after the training program match it and see if it exceeds expectations. Does anyone have a format on this or a checklist of questions?

The Pre-training Questionnaire would differ from training to training and you need to customize it according to the training. However you need to understand the basic objective behind collecting the same.
1. to design the course content
2. to understand the effectiveness of training/ trainer.
3. Scope for future training etc.,
