Hi friends, I have a doubt regarding the ESI Accident Report. In our company, an incident occurred yesterday, but we have not yet submitted the Form 12. Should the Form 12 be submitted to the ESI branch office offline or online? Please provide assistance.

If the Form 12 is submitted late, what problems may arise? Your help is appreciated.

Hi Mr. Prabuap,

Online only, we have to submit the accident report. Once you log in to your ESIC Portal, on the left-hand side, the top option will be the accident report (Form 12). After filling out the report, you need to take a hard copy and give it to the employee. They need to bring it along with their company ID card and visit the ESI office. If you submit it after 48 hours, you won't be able to claim anything from ESIC. Please do it as soon as possible.
