Once data is filled, it cannot be corrected in Form 12 on the ESI Portal by the employer. Therefore, in case of any modifications needed in Form 12, please submit a request letter to the ESI Regional Office along with details of the corrected data. The ESI Regional Office will correct the Form 12 after receiving evidence/proof from the employer.
