Two important clauses need to be incorporated. One, Date of birth declared by you at time of appointment is final & will not be changed. (This to care of employees who produce birth certificate showing older date than declared to increase length of service.) Two, Whenever you change residence, address of which is the one declared by you, you will inform the same in writing immediately on doing so. (Should org want to write official letter, this clause will avoid all related complexities.)
