A death certificate is to be issued only by the competent authority, i.e., the Registrar of Birth and Death for the area where the person died. Normally, this authority is the Local Body Administration. If you are the superior of the deceased, kindly ask their family to submit the death certificate for the purpose of reporting onwards and to facilitate the distribution of the terminal benefits of the deceased employee.

From your post, it seems that you need to issue a service certificate to the member who passed away while in service. All the columns in the regular service certificate will remain the same, with the only difference being the reason for leaving, which will be noted as death in service.

Your query is not clear. A death certificate will be issued by government authorities only. You can't issue any death certificate to his family. You can ask them to submit the death certificate along with the legal heir certificate to the deceased employee's family to settle his account and for EPF and EDLI claim purposes.
