Can you please provide me one form/template to get sign from employee after hand over any material or device to employee? Like i am giving laptop to my employee so i need signed copy that he agreed to my conditions and work for me.

For handing/taking over of company assets what you need is comprehensive "Asset Allocation Policy". Under the provisions of this policy, you need to design "Asset Allocation Form". You need to obtain signature of the employee while allocating the assets. You need to renew the allocation after every quarter or half-year.
