Dear Abhishek,
You have to fill the following documents:
1) Form No. 2 (Renewal of License): I have marked in yellow for you and filled in all necessary details.
2) Treasury Challan Form: Please inquire about the fees as they may vary from state to state. I am also attaching the AP challan.
3) Covering letter: State which documents you are providing and include fee details.
All documents should be submitted to the Factories Inspector.
