Form E - Muster Roll (if Working Time is differed) Form F - Muster roll (if Working time is same) Form M - Salary Register Form N - Service Register Form B - Register of fines Form C - Register of Deduction for damages. Form D - Register of Advance You have to display the following in your establishment notice board: Abstract of Shop and establishment Act, Weekly Holiday, Form J (employer name, working hour, holiday etc.) "Here is no Child Labour" Quote Regd. Hi, I Dont know about tamilnadu shops and establishment act, but under shop act, in pune city following statutory report are maintained. 1. Muster Roll 2. Previsous Month PF Slip 3. Previsous Month ESIC Slip 4. IF ESIC is not Applicable then Workmen Compensation Policy 5. Attendance Register is must. and other supported papers, In above statutory reports some supported documents are common everywhere. Thanks, Bhanudas

How can i claim,
One of the worker in our factory. he went to home due to notfelling well. afterthat two days he joined in private hospital. Afterthat He went to esi hospital for treatment ,But ESI hospital told concerened doctors not availble so kindly join in govt.hosipital in tirupathi.but the worker not intrersted join in govt. hositial . He join the private Hosipital .Now he came to factory and joined in his duty. he would like to reimbruse the hospital bills from ESI.
Now , i woukd like to know how to do.
kindly suggest me. and what are the requrire the documents to claim.
