Is it mandatory to fill up the previous employment details at (10,11 &12), If response is to any or both of (8) & (9) is YES in declaration PF form 11.
Previously i worked two company. Presently i am in third company
My first company deducted PF only one month for me and alloted PF number. After than i quit that job. Presently in my hand that first company PF number available
Second company deducted PF. I withdrew that EPF & EPS amount at the time of leaving.
Now i am working in third company. Here employer is asking to fill up and give declaration PF form 11 (New).
In this form,even if i withdrew EPF & EPS amount of my second company, Is it mandatory to fill up the previous employment details at (10,11 &12), If response is to any or both of (8) & (9) is YES in declaration PF form 11.
Otherwise should i need to write my first company EPF number. But first company deducted PF only one month.
What can i do now.

If you have closed the second company PF, that account will not be alive and then please do not show that account details in your form 11 submitted to the present employer. At the same time, since you have not withdrawn the first PF, you can give details of that account in form 11.
The form 11 attached by you is not relevant now. The EPFO has issued another form 11 in place of form 11 (New) and you have to fill in and keep the new form. I am not sure when will the EPFO withdraw this form also because for EPFO this has become a practice and they are finding new ways of giving more work to HR persons by means of KYCs, UAN etc and they feel that these HR persons are deployed just to do the work connected with EPF and do not have any other work in HRD.
