Dear Mohan,
In Haryana you are to get your establishment registered under the Punjab Shops & Commercial Establishment Act, 1958 as applicable to State of Haryana under section 13 of this Act within one month of the start of the business. For this we are to submit the Form F (3 copies), Form A (2 copies) & Form B (2 Copies) along with the Treasury challan for the fees as required, if any, with the area Labour Inspector who are vested with the powers of issue of registration certificate under the Shops Act.You may have these forms from the city book-sellers who used to keep the registers under the Factories Act.
