just log in to www.epfindia.com where u can EPF forms & details
and www.esic.com for esi..
for EPF u have to calculate 12% of basic + DA but in some companies they provide only basic, but it must covered as per minimum wages act of that state. the maximum limit is 6500/- than it is th employer laibality to provide PF or not. If you have those employees whose basic + Da is more than 6500/ than while calculating EPS u have to limit till 541/- and the remaining employer contribution as to be remited in EPF fund. the form no 2 as to be send for declaration and while the returns i.e before 25 of presiding month u have send the details in form no 5.
and for ESI form no 1 is declaration in which u have put post card size photo, which will initiatally for 3 months. along with that u have send the details in form 3 and even clearly mention or catogarise male & Female. than after 3 months u have submit the details in the card for the permanant insurance no. Every month before 21 st u have to remit the emploee & Emplooyeer contribution to the consent bank and simultaineously the half year returns with priscribed forms i.e before before april 12 & Nov 11 and the maximum amt is 10000/- per month
