Withdrawing/transferring PF Amount From Inoperative Account

Sir, please help. Excellent objective information from you.

I request your advice on the following problem in bullets. Thanks in advance; it's urgent:
- Inoperative A/C since March 2009, dormant for approximately 6 years (excluding service time, i.e., left in 2009 and worked for approximately 2.5 years).
- Company has PF Trust (registered in Mumbai); I'm in Delhi, and the work location was Delhi.
- As I left before 2011, online access is not available. EPFO website shows not updated information and a name mismatch.
- Currently not employed for the last 2 years due to an accident, and the last company did not provide PF.
- How to proceed for withdrawal?
- Will it be taxed (unemployed currently for over 7 years)?
- What about EPS pension part, and how to withdraw?
- Company registered office in Mumbai, and EPFO registration? But I worked at the Delhi branch. Also, the current location?

Please advise.
