Since it has just been a month that I have taken the responsibility.....There is already a leave application letter like the one you have drafted already in place.
The reason I have the Card is that the employee and I have a record of the leaves taken & the balance since the accounts & admin Dep are based in another location and they were in charge of the attendance activities.
