Hi Anu,
If you prepare a proforma Salary slip on your own, who will sign them and without the signature of the Pay roll officer I don't think any employer would recognise this as unauthentic document.
On the other hand, if you have their concurrence to prepare and your Pay roll officer can sign those slips then it may be possible. If this is ok then you may adopt any of the attached formats convenient to you for the purpose. Don't try to fake those slips and produce, if they verify the genuinity you get caught. Be careful.

Dear all,
It is to be noted that the format of Salary Slips should be in the prescribed formats in the applicable/respective labour laws. Therefore before finalising the format you should refer those Acts/Rules in order to not to commit mistake and compliance of contents as prescribed in this regard. For e.g. Code on Wages, state's Shops & Commercial Establishment Act/Payment of Wages Act/Rules etc. Moreover signing and issuing the salary slips individually as many would require them to apply for bank loan etc. And those units following CTC pattern of emoluments, care may be taken for appropriate action.
