While I am happy that you are concerned on the trend as an HR person but your comments that it puts you in poor image in front of Management / operations is a reflection that your organisation is operating at segmented level.
You need to understand and make all managers in the organisation understand that all the activities whether it is recruitment or performance of people is joint responsibility of all the people in the organisation and not just 1 person / department / function.
You need to work on multi prong strategy.. educate and involve management / business manager into talent acquisition efforts so that they understand the nuances of the process and touch base with the reality. This also provide opportunity for the prospective employee to get first hand experience of the culture of your organisations through people with whom he/she will have to work with when they join.
While is critical to hire people with right skills. more important to hire people with Best cultural match. Skills can be learnt in short time, cultural match is difficult and thats the reason they run away after few days. These people do not find culture match with their expectations and therefore run away. Leave is just an excuse.
As suggested by people earlier, you need to dig deep. continuously seek feedback from the people whom you interview (whether selected or rejected) about their experiences with your organisation to understand their expectations.
Remember one may forget everything but one always remember their first interaction with you and specially their first day in the organisation. It is a Moment of Truth for them and you have to ensure that they have positive moment of truth, This positive reinforcement will help them to attach with you better. This is not just a HR issue. Every person they come in contact will have to ensure this. By this you create a culture of reception in your organisation which helps people to gel well together and make new comers feel comfortable in new environment.
The detailed study and data is important to see the pattern so that you have right information and patter to take appropriate action.
