first of all you need to be clear that what do you need. Its appointment letter or a JD.
For any organization, appointment letter has only 1 format for all the employees across the organization. Only the designation, grade, department, location gets changed. Other than that salary structure gets changed.
Regarding JD of procurement manager, you can go through google and find some good points on it.

The Job duties and responsibilities differ from one company to another company. What I suggest that you that you talk the person to whom the Procurement Manager is reporting and set the Key Result Areas and Time bound. You can discuss the same to the New Joinee.
I do agree with Nipun regarding the Appointment Letter.

But for VP level or upper management we want some other format. and i dont have any experience for vp level appointment letters .thats y i want that please share some appointment letters formats.

Please go through portals like Naukri.com where reputed companies advertise for
senior positions with clearly mentioning the JDs for that position.
Please go through and prepare appointment letters.
For senior positions, you need to attach the working sheet of his remuneration or CTC
with a list of other perquisites which he shall be enjoying in your company.
JD should be made separately and attach with the appointment letter.
Please also attach an undertaking to be signed & returned by him regarding Conflict of Interest.
