Hi Paras,
I got the oppprtunity only twice..but were gud learnings.
1. Never use the word 'I',specially when you are in a team.Always, include your colleagues/fellow memebers while describing your achievements.
(What I did in last appraisal)2. Write only what you have actually done.Do not try a language which shows that you are actually just boasting about yourself.If you need to have some kind of training /learn something related to your job,then must frankly submit the same..this helps in a better evaluation.If had some shortfalls /failures,do not exaggerate but if required,then do mention them.
