Actually, I am HR Trainee. I want to know how we apply ESIC for employees and what are the documents needed for the ESIC and finally how we claim or how we use that features. So please did anyone explain my doubts.

During the payment, you need to generate the ESI number of the employee. During that time, you need to enter the details of the employee. That details you can collect from the employee through Form 1.
Take a printout of the Temporary Card and give it to the employee to check the details and do the necessary correction and give a fresh print out. Ask them to attach a family photo (small size) and get the form back. Attest the photograph with Authorized signatory (with seal) and return it back to the employee.
Advice the employee to goto the nearby ESI Branch Office along with the temporary card to do the Biometric Registration. Once it is done, within 30 days, the employee will receive 2 ESI permanent card. Advice the employee to give one card to the family and ask their family members to do the same biometric registration process by holding the ESI permanent card. (There is no need that employee should accompany with them).
Claim: The simple procedure is the employee should visit the respective ESI dispencesary and get the approval for medical reimbursement or leave salary (or both).
For medical reimbursement, the employee should get the referral letter before getting the treatment or within 2 days from the admission date. If the employee got the referral letter before the treatment, then all the payment will be paid to the hospital directly by ESI. If the employee get the referral letter after the admission, he/she can only claim for bills after the payment. During the claim, the employer should give a letter stating that they have not given any financial support to the employee during the treatment. The employee should submit the referral letter, letter from employer and medical bills to the ESI Branch Office.
For Leave Salary, the employee should get a leave letter from ESI. They will provide the leave letter only confirming the sickness. After the leave, the employee should submit the leave letter to the ESI. ESI will ask the employee to get signature from the employer in a leave confirmation form. The employee should submit the filled form to the ESI along with the employer signature. The ESI will pay the leave salary to the bank account of the Employee.

if he or she had came to work for only 10 days in april month he gets salary only 8000 is he applicable for esic and whether he is covered under esic
for eg : 17000+4900+1095 =23095 gross
he worked 10 days he gets salary in earning 8000 is he applicable for Esic or he is out of coverage
