For the establishment there is no such dispensary but it is for the employees a dispensary is allotted. Therefore, if any employee requires to change his dispensary, a request to change dispensary in the prescribed form ( I do not remember the exact form) shall be made to the Local Office of ESI Corporation. Please contact the Manager, ESI Local Office for details.

as Madhu sir said if u want to change the dispensory of any employee use form 53. or else u want to change local esi office, in that case u have to give application to both the office current local office & new local office for tranferring ur records.
for more on this pls consult with esi people or u must have contacts with any esi official.

Employee Contr-1.75%of Gross salary
Emplyer Contr-4.75%of Gross salary
