One of our driver engaged through manpower agency had committed silicide. He has the Provident Fund & ESIC enrollment number and paid his monthly contribution regularly. The dependent of the deceased supplied his death certificate and legal heir certificate to through manpower supply agency . To get his PF deposited amount , EDLI dues and family pension along with any other benefit from ESIC what are the procedure to be followed, please guide us.

You need to fill up EPF Composite Claim Form (Form 20 for pf payment , Form 10D for pension , Form 5IF for EDLI ) and signed by the employer along with the claim documents and submit to the EPFO office .
For ESIC need to fill up funeral expenses form and get signed by the employer and submit to respective ESI Office.
