Form 10 D is the application form for the monthly pension under the provisions of EPF & Misc. Provision Act 1995. It is to be submitted by the first claimant after reaching the age of 58 years, even while still in service, with a minimum of 10 years of service to apply for the pension. The claimant can be a member, widow/widower, orphan, or nominee, depending on the circumstances. The pro forma of the form is enclosed herewith for reference.
